Getting a Blank White Window inside Google Forms

When you launch the email notifications add-on inside Google Forms, you may sometimes get a blank window and none of the text fields and buttons are visible on the page.

To get around this problem, please ensure that third-party cookies are not blocked in your browser.

If enabling cookies does not solve the white screen problem, please try signing out of multiple Google accounts as described in this workaround.
